İstanbul Kültür Üniversitesi Mimarlık Fakültesi'ne Mazeret Raporu Teslim Edecek Öğrencilerin Dikkatine;
     For the Attention of Students who are going to submit
     
Medical Report;
Sağlık Raporu Teslim Süreci sırasıyla aşağıdaki gibidir:
      The submission process of Medical Reports is as follows:

1- İlgili Sağlık Kuruluşundan alınan raporların rapor bitiş tarihinden itibaren 5 iş günü içinde İKÜ Sağlık Kültür ve Spor Dairesi Başkanlığı’na onaylatılması
1.    To get the approval of the Directorate of IKU Health Culture and Sports Department on the reports received from the relevant Health Institution within 5 working days starting from the final date of the medical report.
2-İstanbul Kültür Üniversitesi Mimarlık Fakültesi web sayfasından temin edilen Öğrenci Rapor Formu’nun, raporun kapsadığı tarih aralığında öğrencinin katılamadığı ders ve/veya sınavlar dikkate alınarak eksiksiz doldurulması ve öğrenci tarafından imzalanması
2.    To complete and sign the Student Report Form on the website of Istanbul Kultur University Faculty of Architecture, taking into consideration the courses and/or the exams that the student could not attend during the period of the medical report. 

3-Doldurulan Öğrenci Rapor Formunun Bölüm Sekreterliğine rapor bitiş tarihinden itibaren 5 iş günü içinde onaylatılması,
3.    To get the approval of the Secretariat of the Department on the completed Student Medical Report within 5 working days starting from the final date of the medical report.
4- Öğrenci tarafından Sağlık kuruluşundan alınan onaylanmış Rapor ile onaylanmış Öğrenci Rapor Formunun (her ikisinin de) aslının Bölüm Sekreterliğine, fotokopilerinin de ilgili ders yürütücülerine veya ilgili ders asistanlarına elden teslim edilmesi gerekmektedir.
4.    The student must submit the original approved Report received from the relevant Health Institution and the approved Student Report Form (both of them) to the Secretariat of the Department  and their copies to the related course instructors or the related course assistants. 

Ders yürütücüsüne ulaşmayan raporlar işleme alınmamaktadır.
Reports that do not reach the course instructors are not put into process.
